You have to call your provider, which I am assuming is T-Mobile since the t629 is a T-Mobile phone. Their customer service is open 24/7, and can be reached at 1-800-937-8997. They are the only people that can give you the PUK code. They will verify that you are the account holder and ask for some info from the phone and sim card, then get the code for you. If you are not the account holder, then you will have to get the account holder to call.
